Position Description: |
Responsibilities include engaging youth, families, natural supports, and system partners in collaborative planning; monitoring service implementation and progress; identifying and addressing service gaps; coordinating re-entry planning for youth returning from out-of-home placements; maintaining timely documentation and case records; tracking outcomes and program data; and assisting families in accessing supportive funding and resources. The successful candidate will demonstrate a commitment to family voice and choice, strength-based practice, community collaboration, and unconditional care. This position also serves as a liaison with community organizations and public agencies, participates in program evaluation and quality improvement efforts, attends required meetings and trainings, and supports the agency's mission of improving outcomes for children and families. |

Brief Description: |
Licking County Children & Families First Council, a division of Licking County Job & Family Services, is seeking a Wraparound Facilitator. This position coordinates and supports individualized, family-driven services for youth with complex behavioral health, child welfare, and multi-system needs. This position leads the wraparound planning process by facilitating team meetings, developing safety and crisis plans, coordinating services across agencies and community partners, and ensuring families receive the support needed to achieve positive outcomes. |

Job Prerequisites: |
The successful candidate will possess a bachelor's degree in social work, psychology, sociology, human services, or a closely related field. Experience working with children and families facing challenges related to child welfare, behavioral health, substance use, family stability, or crisis intervention is preferred. Knowledge of community resources and service systems is highly desirable. Additional qualifications include: - Strong interpersonal, communication, and problem-solving skills.
- Ability to effectively engage and work with families experiencing complex needs and challenging circumstances.
- Ability to work flexible hours.
- Ability to maintain professionalism and composure when interacting with individuals who may be upset, distressed, or in crisis.
- Ability to travel throughout the service area; driving is an essential function of this position.
Candidates must possess a valid driver's license and maintain an acceptable driving record in accordance with County policy. Employment is contingent upon the successful completion of a background check and drug screening. |
